Injuries to the lower lumbar intervertebral discs, degenerative disc disease preexisting or both, are responsible for many lower back problems, labor or otherwise. Symptoms appear at any age, with a peak incidence in the third or fourth decade. Nearly half the population under 65 years has had a debilitating back pain. At 60 years of age two thirds of adults have some radiographic evidence of degenerative disc disease.

The discs become less elastic with age progressively, and degeneration can cause the ring to curl back toward the spinal canal. The nucleus pulposus of the ring can also bulge or herniate through a weak portion of the ring, or be downright sequestered in the spinal canal. The pressure of a disk or fragment thereof on a nerve root causes pain or weakness in the distribution of sensorimotor nerve root. The cause of pain in the absence of direct pressure on the nerve root is not well understood, may be due to inflammation in response to injury of the degenerated disc. There are free nerve endings in the posterior third of the ring.

Clinical Manifestations

the onset of symptoms is gradual and sudden. Patients sometimes arise with back pain after a day of vigorous activity, or the symptoms are directly related to specific fall or a slip catch. Differential Diagnosis

Almost any disease process that affects column, meninges, abdomen, pelvis or retroperitoneal area causes pain in the lower back. The physical examination should include evaluation of costovertebral angle tenderness, urinalysis to rule out renal, abdominal exploration to rule out aneurysms and peripheral pulse evaluation to look for causes vascular pain. Symptoms usually occur in subjects whose occupation includes the performance of repetitive wrist movements, which can cause inflammation of ligament and nerve compression, although other causes such as fractures or esapcio occupying lesions may also be at the origin of its development.
